Scope of Position The Advanced Practice Provider (APP) Lead works with APP and Division leadership and will function in liaison, oversight, and mentoring roles for other advanced practice providers. Administrative and reporting responsibilities as per department expectations. Interacts with the division workforce at large on issues related to advanced practice providers. Assigned responsibility for mentoring advanced practice providers and is designated expert for advanced practice in the division. Provides clinical and day-to-day leadership and mentoring in matters relating to advanced practice and the professional development of APRNs or PAs for serving as the liaison between the APRN/PAs and upper-level advanced practice management (APP Manager and APP Director). The APP Lead is responsible for providing health care services to patients within the parameters of a standard care or supervisory agreement with an attending physician member of The Ohio State University Wexner Medical Center. Collaborates with a physician (s) and other clinicians in providing a full scope of patient care. The focus is on preventive health or the management of acute and/or stable chronic conditions. Performs medical services that have been specifically authorized and directed by the credentialing body and the certifying organization through which they hold licensure. Position Summary The APP Lead provides health care services to patients working in collaboration with a licensed physician member of the medical staff. Healthcare services provided include care during acute and chronic phases of illness, education, and counseling of individuals and families, health promotion and prevention, and referral to other healthcare providers and community resources when appropriate Minimum Qualifications Current certificate of authority to practice as an APRN in the State of Ohio Graduate of an APRN program that qualifies the graduate to sit for the certification examination of a national certifying organization recognized by the Ohio Board of Nursing Masters Degree in Science or Nursing OR Masters Prepared; Graduate of a PA-C program that qualifies the graduate to sit for the certification examination recognized by the National Certification Commission for Physician Assistants (NCCPA).   • Evaluates new and existing patients. Assesses the physical and psychosocial status of patients by means of
  • interview, health history, physical examination, laboratory studies, and diagnostic studies
  • Works in collaboration with a physician in formulating treatment plans for health problems and follow-up.
  • Performs and/or assists with diagnostic procedures as outlined in the SCA or SA.
  • Administers therapeutic measures/obtains specimens as prescribed.
  • Interprets and evaluates findings of studies/tests. Schedules diagnostic and operative procedures.
  • Recognizes deviations from normal in the physical, laboratory, and radiographic assessment of patients.
  • Relays appropriate information regarding patient care to the collaborating physician.
  • Prescribes medication and blood products based upon laboratory results, and initiates orders for routine diagnostic and follow-up studies, therapeutic measures, and care post-discharge in accordance with written practice protocols.
  • Initiates appropriate actions to facilitate the implementation of therapeutic plans consistent with the continuing health care needs of clients.
  • Evaluates the quality of care provided and recommends changes for improvement in the delivery of care.
  • Triages patients and provides emergency treatment when appropriate
  • Writes patient history, admissions care plans, progress notes, and discharge notes. Evaluates new patients, develops treatment plans, and makes changes to the treatment plan for existing patients.
  • Documents and promotes appropriate documentation by the health care team.
  • Sends follow-up letters to referring physicians with summaries of treatment, patient response, and plan of care
  • Requests records and written consultations from physicians and other health care professionals to ensure patient care is appropriate.
  • Provides preventative health services for patients (screening, risk assessments, immunizations, etc.)
  • Evaluates new inpatient consults and communicates the treatment plan to the primary service.
  • Assists the service in the coordination of discharge plans of patients and directs arrangements for subsequent follow-up in a clinic or with other health care providers, home care agencies, pharmacies, or referring physicians
  • Establishes a network of referral sources to serve as clinical resources for the APP, the collaborating physician and the patient population.
  • Maintains contact with and services as a consultant to home care agencies.
  • Assists in the management of family dynamics and coping mechanisms during acute and chronic phases of the patients care.
  • Considers the physical, emotional, social, and economic status and cultural and environmental backgrounds of individuals, families, and communities when providing health care.
  • Duties as assigned 10% Administrative Duties/Responsibilities:
    • Serves as Leader for APRNs and PA-Cs within OSUMC as directed by the APP Manager and Director
    • Leads day-to-day activities of APPs including scheduling, onboarding, amp; contributing to the annual performance review in conjunction with the APP Manager
    • Interacts primarily with peers and subordinates to share information in order to resolve routine matters
    • Develop, administer, and oversee education and assessment of clinical competencies for APPs they support
    • Assist with data collection and analysis of Quality Metrics and Patient Satisfaction
    • Leads multidisciplinary teams, as assigned, in the development of ongoing progress of clinical outcomes to achieve organizational objectives
    • Participates in the review and revision of job descriptions, policies and procedures, and standards of care for the overseeing body of Advanced Practice. Duties to remain consistent across the medical center.
    • Explains policies, standards, and processes to peers and subordinates
    • Ensures that departmental and intradepartmental policies, procedures, and standards of care are maintained and assists in interpreting these to others
    • Acts as a role model and resource person for administrators, staff, and the community.
    • Participates in quality improvement activities on an ongoing basis.
    • Duties as assigned 5% Organizational/Departmental Duties/Responsibilities:
      • Helps support and is accountable to the development of ongoing progress of clinical outcomes to achieve organizational objectives,
      • Ensures that departmental and intradepartmental policies, procedures, and standards of care are followed and assists in interpreting these to patients and visitors.
      • Acts as a role model and resource person for administrators, staff, and the community.
      • Ensures and maintains patient/family confidentiality, privacy, and patient rights at all times.
      • Informs management and/or Nurse Executives of actual/potential problems that require resolution at the administrative level.
      • Participates in quality improvement activities on an ongoing basis.
      • Bills all revenue-producing encounters as directed by their compliance department specialist.
      • Adheres to the credential/privilege process in order to maintain clinical privileges
      • Participates in multidisciplinary and nursing research activities.
      • Acts as a patient advocate to ensure the protection of human rights.
      • Applies current concepts and findings from research and studies to practice.
      • Responsible for the formal and informal education to APRN/PA staff.
      • Participates in the development of education materials for patients and health care providers.
      • Maintains current knowledge of health care techniques and practices by participating in continuing education opportunities and professional organizations.
      • Acts as a role model in appearance, demeanor, and actions to APRN/PA staff.
      • Duties as assigned Patient Population Served Knowledge of growth and development and an understanding of the range of treatments necessary to meet the age-specific needs of the patient population served:
        • Neonates (0 - 6 months)
        • Children (7 months - 13 years)
        • Adolescents (14 17 years)
        • Adults (18 - 64 years)
        • Geriatrics (65 + years)
